We are providing this list of FAQs to help our patients better understand just what a family fractice is and how we fit in to the full range of medical services available under our health care system. This list is far from conclusive, so if you have any questions - particularly about your specific medical care or diagnosis, be sure to discuss it with your Perimeter North Medical Associates physician or nurse practitioner.

What is a family practice?
Family medicine is the branch of medicine that is concerned with the diagnosis and treatment of health problems in people of either sex at any age. Practitioners of family medicine are often referred to as primary care providers (PCP).

What is a nurse practitioner?
Nurse practitioners are registered nurses who have earned a master's degree in nursing. Our nurse practitioners are board certified in family medicine. This allows them to diagnose and treat patients of all ages.

What if I have an emergency or need to contact a doctor after hours?
If you need immediate medical attention, please call 911 or proceed to an emergency room. To reach our on-call provider please call any of our offices and follow the prompts to leave a message for a provider.

  • Perimeter North Medical Associates's policy does not permit us to call in prescriptions for narcotics or pain medication after hours.
  • Generally, we will not call in antibiotic prescriptions without first seeing you for the problem.
  • Please call during regular office hours to make an appointment, request a referral or to have a refill request processed.
  • Please follow your insurance policy's guidelines regarding after hours care. Failure to do so may leave you liable for any charges that are incurred.

Does Perimeter North Medical Associates perform immigration physicals?
Dr. Komal Balaney, Dr. Sailaja Gadde and Dr. Reshma Shah in our Suwanee/Johns Creek office have recently been appointed Civil Surgeons by the U.S. Citizenship and Immigration service. This means they are each authorized to perform the physical examination required to complete the Report of Medical Examination and Vaccination Record (I-693 Form). If you need this service performed please call our Scheduling Department to have them set up an appointment for an Immigration Physical.

Before coming to your appointment, download the I-693 form from the U.S. Citizenship and Immigration service website and fill out Part 1 completely. Please note a separate I-693 form and examination is required for each applicant. When coming to your appointment each applicant must also present a government issued photo identification. For applicants under 14 years of age other proof of identity that shows name, date and place of birth and parent's full names is acceptable.

I need a referral to a specialist. How is that handled?
Referrals are handled through each office location. Please contact them directly and follow the prompts.
